On January 28, 1986, the Space Shuttle Challenger broke apart 73 seconds after launch, killing all seven crew members. The cause was eventually traced to the failure of an O-ring seal in the right solid rocket booster, which became brittle in the unusually cold 36°F (2°C) weather at launch. But here's the haunting part: engineers at Morton Thiokol, the booster manufacturer, had data showing that O-ring damage was linked to cold temperatures, and they had argued against launching the night before. The problem, as Edward Tufte later demonstrated in his 1997 book 'Visual Explanations,' was not a lack of data but a catastrophic failure of data presentation. The engineers' charts scattered O-ring incident data across 13 separate rocket-by-rocket displays, listed in order of launch date rather than temperature, making it nearly impossible to see the critical temperature pattern. Tufte showed that if the same data had been plotted on a simple scatter plot — O-ring damage versus launch temperature — the relationship would have been immediately and terrifyingly obvious: every launch below 66°F showed damage, and the proposed launch temperature of 36°F was far outside any previously tested range. The Challenger case became Tufte's most powerful and most tragic example of how bad information design can have life-or-death consequences, and it remains a standard case study in engineering ethics, decision science, and data visualization courses worldwide.
